RESUMO

Objective: To construct the diagnostic model of superficial esophageal squamous cell carcinoma (ESCC) and precancerous lesions in endoscopic images based on the YOLOv5l model by using deep learning method of artificial intelligence to improve the diagnosis of early ESCC and precancerous lesions under endoscopy. Methods: 13, 009 endoscopic esophageal images of white light imaging (WLI), narrow band imaging (NBI) and lugol chromoendoscopy (LCE) were collected from June 2019 to July 2021 from 1, 126 patients at the Cancer Hospital, Chinese Academy of Medical Sciences, including low-grade intraepithelial neoplasia, high-grade intraepithelial neoplasia, ESCC limited to the mucosal layer, benign esophageal lesions and normal esophagus. By computerized random function method, the images were divided into a training set (11, 547 images from 1, 025 patients) and a validation set (1, 462 images from 101 patients). The YOLOv5l model was trained and constructed with the training set, and the model was validated with the validation set, while the validation set was diagnosed by two senior and two junior endoscopists, respectively, to compare the diagnostic results of YOLOv5l model and those of the endoscopists. Results: In the validation set, the accuracy, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of the YOLOv5l model in diagnosing early ESCC and precancerous lesions in the WLI, NBI and LCE modes were 96.9%, 87.9%, 98.3%, 88.8%, 98.1%, and 98.6%, 89.3%, 99.5%, 94.4%, 98.2%, and 93.0%, 77.5%, 98.0%, 92.6%, 93.1%, respectively. The accuracy in the NBI model was higher than that in the WLI model (P<0.05) and lower than that in the LCE model (P<0.05). The diagnostic accuracies of YOLOv5l model in the WLI, NBI and LCE modes for the early ESCC and precancerous lesions were similar to those of the 2 senior endoscopists (96.9%, 98.8%, 94.3%, and 97.5%, 99.6%, 91.9%, respectively; P>0.05), but significantly higher than those of the 2 junior endoscopists (84.7%, 92.9%, 81.6% and 88.3%, 91.9%, 81.2%, respectively; P<0.05). Conclusion: The constructed YOLOv5l model has high accuracy in diagnosing early ESCC and precancerous lesions in endoscopic WLI, NBI and LCE modes, which can assist junior endoscopists to improve diagnosis and reduce missed diagnoses.

RESUMO

Type 1 diabetes (T1D) is an autoimmune disease that resulted from the severe destruction of the insulin-producing β cells in the pancreases of individuals with a genetic predisposition. Genome-wide studies have identified HLA and other risk genes associated with T1D susceptibility in humans. However, evidence obtained from the incomplete concordance of diabetes incidence among monozygotic twins suggests that environmental factors also play critical roles in T1D pathogenesis. Epigenetics is a rapidly growing field that serves as a bridge to link T1D risk genes and environmental exposures, thereby modulating the expression of critical genes relevant to T1D development beyond the changes of DNA sequences. Indeed, there is compelling evidence that epigenetic changes induced by environmental insults are implicated in T1D pathogenesis. Herein, we sought to summarize the recent progress in terms of epigenetic mechanisms in T1D initiation and progression, and discuss their potential as biomarkers and therapeutic targets in the T1D setting.

RESUMO

3D printing technology has been widely applied in the field of clinical education in vascular surgery due to the advantage of 100% reduction of objects. Vascular surgeons with different levels benefit from 3D printing application in anatomy structure, operation plan, skill training with simulator. At present, the application of 3D printing model in vascular surgery is still in the initial stage with some limitations. This paper reviews the application, limitations, and prospects of 3D printing model in clinical teaching of vascular surgery.

Objective To evaluate the value of MRI-T2WI texture analysis in the differentiation of atypical medulloblastoma and ependymoma of the fourth ventricle.Methods Preoperative MRI data of 36 cases of fourth ventricle tumor (19 cases of medulloblastoma and 17 cases of ependymoma) confirmed by the Central Theater General Hospital of the Chinese People's Liberation Army were retrospectively analyzed.Manually sketch areas of interest (ROI) were made using texture analysis software to get histogram parameters,including mean,median,standard deviation,skewness,kurtosis,maximum,minimum,heterogeneity,entropy,the 5th percentile (T2WI5th),the 10th percentile (T2WI10th),the 25th percentile (T2WI25th),the 50th percentile (T2WI50th),the 75th percentile(T2WI75th),the 95th percentile (T2WI95th).Independent sample t test or mann-whitney U test was used for statistical analysis of each parameter value between the two groups.Logistic regression analysis was used to perform multiparameter joint analysis on meaningful parameters.The area under the curve (AUC) was obtained by using the subject operating characteristic (ROC) curve to determine the threshold and diagnostic efficacy for distinguishing the two group.Results The mean,median,T2WI5th,T2W10th,T2WI25th,T2WI50th,T2WI75th values of the medulloblastoma group were smaller than those of the ependymoma group,and difference was statistically significant (P < 0.05).The T2WI5th had the best diagnostic performance.When the critical value was 1 343,the diagnostic specificity was 75.0% and the sensitivity was 92.9%.Logistic regression was used to predict the joint analysis of probability parameters.T2WI15th+T2WI10th had the best performance,and AUC was 0.928.When the critical value was 0.75,the diagnostic specificity was 75.0% and the sensitivity was 100.0%.Conclusions MRI-T2WI texture analysis can provide quantitative information for preoperative diagnosis of atypical medulloblastoma and ependymoma and improve the accuracy of preoperative diagnosis,which has obvious clinical value.

Breast cancer is one of the common diseases among women. It is a malignant tumor with a variety of complex mechanisms. Its pathogenesis has not been clearly studied. Physical, chemical and surgical treatments often cause vomiting, nausea, dizziness and headache for women. As compared with traditional treatment, Chinese medicine is characterized by multiple targets, small side effect and good effect in treating breast cancer. In this paper, 85 kinds of Chinese herbal medicines that can treat breast cancer were included. Among them, 69 kinds of Chinese herbal medicines have been included in the 2015 edition of the Chinese Pharmacopoeia, and 16 kinds of Chinese herbal medicines have not been included. The main medicinal ingredients in these Chinese herbal medicines for treatment of breast cancer were alkaloids, glycosides, phenols, terpenes, carbohydrates, volatile oils, coumarins and so on. In addition, these herbal medicines were classified according to their effects in breast cancer. Then, combined with the recent studies at home and abroad, this paper summarized the effect of traditional Chinese medicine(TCM) on breast cancer, including the reversal of multi-drug resistance, the inhibition of metastasis and proliferation, the induction of tumor cell apoptosis, and the arrest of the cell cycle for breast cancer. This paper also explained three pathways for treating breast cancer by TCM, including intervening the tumor cell related apoptosis gene to inhibit breast cancer, inhibiting the expression of P-glycoprotein in the cell membrane to reverse the multi-drug resistance of breast cancer cells, and regulating the related epithelial mesenchymal transition signal pathway to prevent breast cancer cells metastasis and proliferation. In the end, it was concluded that Chinese medicine can reduce the drug resistance and metastasis of breast cancer cells, block the cell cycle of breast cancer cells, and also intervene the expression of apoptotic factors to promote the death of breast cancer cells. The inhibition of breast cancer by Chinese medicine was the result of the common effect of various ingredients. Therefore, Chinese medicine treatment for breast cancer has the unparalleled advantages as compared with chemical and surgical treatment. Chinese medicine is one of our important means to overcome breast cancer.

Purpose To explore the expression of bone morphogenetic protein-4 (BMP-4) in prostate cancer tissues and to analyze the effects of its down-regulation on proliferation and invasion of prostate cancer cells. Methods The expression of BMP-4 protein in prostate tissues of 58 patients with prostate cancer and 40 patients with benign prostatic hyperplasia (BPH) was detected by immunohistochemical En Vision method. The BMP-4 protein expression of different prostate cancer cells (LNcap, PC-3, Du145) and BPH was analyzed by Western blot.BMP-4 siRNA and control siRNA were transfected into LNcap respectively and then divided into three groups: untreated group, control siRNA group and BMP-4 siRNA group. Western blot was used to detect the expression levels of BMP-4 protein, MMP-2and MMP-9 in LNcap cells among the three groups. The proliferation of three groups was detected by CCK 8. The invasion ability of three groups was detected by Transwell chamber. Results The positive proportion of BMP-4 protein in prostate cancer tissues was higher than that in benign prostatic hyperplasia (P < 0.05) , and the expression level of BMP-4 protein in three kinds of prostate cancer cells was higher than that in BPH cell. The expression of BMP-4 protein in BMP-4 siRNA group was lower than that in untreated group and control siRNA group.The cell proliferation ability and cell invasion ability in BMP-4siRNA group were lower than those in untreated group and control siRNA group. The expression levels of MMP-2 and MMP-9proteins in BMP-4 siRNA group were lower than those in untreated group and control siRNA group. Conclusion BMP-4 is highly expressed in prostate cancer tissues and is associated with proliferation and invasion of prostate cancer cells, and it may become a potential therapeutic target for prostate cancer.

OBJECTIVE@#To explore the diagnostic value of magnetic resonance imaging(MRI) in anal fistula.@*METHODS@#A total of 2160 patients were clinically diagnosed with anal fistula at the Sixth Affiliated Hospital of Sun Yat-sen University from March 2010 to September 2015. Among them, 232 cases with operative history at other hospital, 218 with Crohn's disease, 6 with rectum cancer and 8 with other disease were excluded, and 1696 patients were finally enrolled and retrospectively analyzed. The saggital FSE T2WI imaging was confirmed based on the midline of body, and then the coronal and axial scanning line were confirmed. The key point was that the coronal scanning line must parallel and the axial scanning line must be perpendicular to the major axis of anal canal. The characteristics of anal fistula were recorded, and anal fistula were classified as five types, including intersphincteric, transphincteric, suprasphincteric, extrasphincteric and superficial fistula according to the Parks classification and our experience. The distribution of internal opening was described by using lithotomy position clock method.@*RESULTS@#Of 1696 patients, 1456 were males and 240 females with median age of 26.5 (0.2 to 87.0) years. Age of 8.4% (143/1696) cases was under 20 years old, of 57.4%(973/1696) cases was between 20 to 40, of 28.4%(482/1696) cases was between 40 to 60, of 5.8%(98/1696) cases was over 60. The 1696 MR examinations included 1128 on 1.5T MR and 568 on 0.5T MR. Of all the anal fistulas was 29.0%(492) high position and 71.0%(1204) was low position. Among the 1696 patients, 1057 were intersphincteric fistulas(62.3%), 407 were transphincteric fistulas(24.0%), 68 were suprasphincteric fistulas(4.0%), 54 were extrasphincteric fistulas (3.2%), 67 were superficial fistulas(4.0%), and 43(2.5%) were difficult to classify. A total of 1996 internal openings were found and most of them were located around the dentate line of 5-7 o'clock in lithotomy position(47.7%, 952/1996).@*CONCLUSIONS@#Anal fistula mainly occur in young men, and the most common type is intersphincteric fistula. MRI can accurately classify anal fistulas and clearly demonstrate internal openings, and provide reliable evidence for clinical treatment and surgery.

Objective To evaluate 3D printing abdominal aortic aneurysm model in analysis of clinical teaching effect for standardized resident doctors in vascular surgery department. Methods 48 resi-dents in vascular surgery department in our hospital from December 2016 to September 2017 were seleeected and randomly divided into control group and the experimental group. The traditional vascular surgical anatomy atlas, ultrasound, CT abdominal blood vessel 3D reconstruction, digital subtraction, video and so on were used by 24 residents in the control group, while in the experimental group, on the basis of the traditional teaching, abdominal aortic aneurysm model of 3D printing, true aortic coated stent delivery system were increased. After the teaching, the theory of evaluation (abdominal vascular anatomical features, morphological characteristics and classification of AAA, measurement of various parameters and key points of operation in the EVAR) and satisfaction questionnaire were adopted to evaluate the effect of two kinds of teaching methods. SPSS 19.0 was used to conduct t test on two groups of physician evaluation data. Results The results of theoretical assessment showed that there was no significant difference between the control group and the experimental group in the abdominal vascular anatomical features, the morphological features and the classification of the experimental group (P>0.05) in the examination of the common AAA cases and the complicated AAA cases. However, the experimental group was higher than the control group in the mea-surement of the parameters of EVAR, and the score of the operation points and the total score, and the difference was statistically significant (t=2.283, t=2.263, P<0.05). The results of the questionnaire showed that the students' satisfaction scores on the teaching satisfaction of the normal and complex AAA cases were better than those of the control group, and the difference was statistically significant (P<0.05). Conclusion The 3D printing model can increase the understanding and mastery of the anatomy and treat-ment of abdominal aortic aneurysm and improve its learning enthusiasm for vascular surgery. We should make full use of the advantages of 3D printing technology on the basis of retaining the advantages of tradi-tional teaching methods and means, and further enhance the teaching effect.

AIM: To investigate the clinical characteristics of patients with systemic lupus erythematosus ( SLE) complicated with retinopathy. METHODS: Totally 121 cases of SLE patients treated in our hospital from February 2015 to February 2017 were selected,including 30 cases with retinopathy(observation group) and 91 cases without retinopathy(control group), the clinical manifestations and laboratory examination results of the two groups were compared. RESULTS: ln the observation group, there were 6 patients with bilateral retinopathy and 24 patients with monocular retinopathy. Cotton retinal exudation, retinal vascular occlusion and retinal hemorrhage lesions were common, accounted for 33%, 25% and 19%. The incidence of skin rash, skin vasculitis and Raynaud's phenomenon in the observation group were 63%, 47% and 37%, respectively, which were significantly higher than those in the control group (P<0.05). There were no significant difference in the incidence of mucosal ulcer, arthritis, neuropsychiatric symptoms and pleurisy between the observation group and the control group(P>0.05). The positive rate of anti ds-DNA in the observation group was 63%, which was significantly higher than that in the control group (P<0.05). The observation group and the control group urine protein > 3 +, anti Sm antibody positive and rRNP positive and antiphospholipid antibody percentage differences were not statistically significant(P>0.05). The SLEDAI score of the observation group was 20.14 (9, 30) points, which was significantly higher than that of the control group (P<0. 05). The modified BenEzra score of the observation group was 10 04士3.15,and was positively correlated with the SLEDAI score (rs=0.706,P<0.05). CONCLUSION: SLE with fundus retinopathy patients mainly manifest as cotton wool spots, retinal vascular occlusion and retinal hemorrhage; rashes, vasculitis, Raynaud's phenomenon,and anti ds-DNA antibodies are common in SLE patients with retinopathy. The modified BenEzra score plays an important role in the evaluation of SLE disease activity and retinal vascular lesions.

Objective To investigate the value of enhanced MR texture analysis in the differential diagnosis of single brain metastasis (SBM) and high grade glioma (HGG). Methods The preoperative MRI images of 21 SBM and 23 HGG patients confirmed by pathology were retrospectively analyzed. The T1WI enhanced images were used for texture analysis to obtain the mean, median, entropy, skewness, kurtosis, heterogeneity and other parameters. An independent sample t-test or Mann-Whitney U test was used to statistically analyze the values of the parameters between the two groups, and ROC evaluation parameters were used to identify the diagnostic efficacy of SBM and HGG. Results There were significant differences in entropy and heterogeneity parameters between SBM and HGG: (4.838 ± 0.523 vs. 5.358 ± 0.449, 0.276 ± 0.066 vs. 0.331 ± 0.101), P<0.05. When the cut-off value of entropy was 4.88, the sensitivity of differential diagnosis between SBM and HGG was 47.6%, specificity was 100.0%, and the area under ROC curve was the largest (0.745). Conclusions The entropy value and heterogeneity have certain value in distinguishing SBM and HGG in MRI texture analysis.

<p><b>OBJECTIVE</b>To investigate the association of leptin, adiponectin, and ghrelin in breast milk with the weight growth velocity of infants with exclusive breastfeeding.</p><p><b>METHODS</b>A total of 67 full-term singleton infants who received regular child care and exclusive breastfeeding and their mothers were enrolled. The nutritional status was evaluated based on the measurements of body weight and body length (underweight, growth retardation, emaciation, overweight, and obesity). Z score was used to calculate growth velocity, and according to the ΔZ score, the infants were divided into poor growth group, low growth velocity group, and normal growth velocity group. Mature breast milk samples were collected from their mothers, and ELISA was used to measure the levels of leptin, adiponectin, and ghrelin.</p><p><b>RESULTS</b>The emaciation group had a significantly lower level of leptin in breast milk than the non-emaciation group (P<0.05), and the overweight/obesity group had a significantly lower level of adiponectin than the non-overweight/obesity group (P<0.05). The correlation analysis showed that the level of ghrelin in breast milk was positively correlated with Z score of current body weight and ΔZ score compared with birth weight (r=0.280 and 0.290 respectively; P<0.05). The regression analysis showed that the level of ghrelin in breast milk was an important influencing factor for the Z score of body weight (β=0.161, P<0.05).</p><p><b>CONCLUSIONS</b>Various active constituents in breast milk, including leptin, adiponectin, and ghrelin, may regulate the growth and development of infants to a certain degree, but long-term studies and observation are needed to investigate their association with offspring growth and development and the health-promoting effect of breast milk on offspring.</p>

<p><b>BACKGROUND</b>Lots of trials demonstrate that second-generation drug-eluting stents (G2-DES), with their improved properties, offer significantly superior efficacy and safety profiles compared to first generation DES (G1-DES) for patients with coronary artery disease (CAD) receiving percutaneous coronary intervention (PCI). This study aimed to verify the advantage of G2-DES over G1-DES in Chinese patients with stable CAD (SCAD).</p><p><b>METHODS</b>For this retrospective observational analysis, 2709 SCAD patients with either G1-DES (n = 863) or G2-DES (n = 1846) were enrolled consecutively throughout 2013. Propensity score matching (PSM) was applied to control differing baseline factors. Two-year outcomes, including major adverse coronary events as well as individual events, including target vessel-related myocardial infarction, target lesion revascularization (TLR), target vessel revascularization, and cardiogenic death were evaluated.</p><p><b>RESULTS</b>The incidence of revascularization between G1- and G2-DES showed a trend of significant difference with a threshold P - value (8.6% vs. 6.7%, χ2 = 2.995, P = 0.084). G2-DES significantly improved TLR-free survival compared to G1-DES (96.6% vs. 97.9%, P = 0.049) and revascularization-free survival curve showed a trend of improvement of G2-DES (92.0% vs. 93.8%, P = 0.082). These differences diminished after PSM. Multivariate Cox proportional hazard regression analysis showed a trend for G1-associated increase in revascularization (hazard ratio: 1.28, 95% confidence interval: 0.95-1.72, P = 0.099) while no significance was found after PSM. Other endpoints showed no significant differences after multivariate adjustment regardless of PSM.</p><p><b>CONCLUSIONS</b>G1-DES showed the same safety as G2-DES in this large Chinese cohort of real-world patients. However, G2-DES improved TLR-free survival of SCAD patients 2 years after PCI. The advantage was influenced by baseline clinical factors. G1-DES was associated with a trend of increase in revascularization risk and was not an independent predictor of worse medium-term prognosis compared with G2-DES.</p>

Cell viability was measured by MTT assay. Cell migration and invasion was measured by wound scratch assay and transwell migration assay,respectively. The expression of matrix metalloproteinase 2 (MMP2), MMP9, transforming growth factor-β(TGF-β1), p-Smad3 was measured by western blot. Results The expression of miR-424 in NCI-H460, NCI-H1975, NCI-H446, A549, NCI-H1299, NCI-H157 cells (1. 78 ± 0. 13, 1. 69 ± 0. 10, 1. 89 ± 0. 18, 2. 88 ± 0. 27, 2. 52 ± 0. 20, 2. 49 ± 0. 23) was significantly higher than that in MRC-5 cell 0. 58 ± 0. 05(P< 0. 01). Compared with the miR-424 NC group, the cell viability was reduced (P<0. 01), cell migration and invasion ability was decreased (P< 0. 01), and the expression of MMP2, MMP9, TGF-β1, p-Smad3 protein was down-regulated in miR-424 inhibitor group ( P < 0. 01 ) . Conclusions miR-424 inhibitor can inhibit the migration and invasion in lung cancer A549 cells via inhibition of TGF-β1/Smad3 signal pathway.

Objective To investigate the difference of retinal nerve fiber layer thickness(RNFL)in opti-cal coherence tomography(OCT)under low and moderate signal strength. Methods Four hundred eyes of people aged 46~75 with clear fundus image,no obvious fundus diseases,and satisfactory optical coherence tomography were classified according to their ages and signal strength. The peripapillary and 4 quadrants of RNFL were detect-ed with OCT. Results In satisfactory OCT images ,the signal strength that reached 4/10 and 7/10 was 1.5% and 27% respectively. Under low and moderate signal strength ,the maximum thickness of RNFL was at the superior and inferior,and the minimum thickness of RNFL was at the nasal and temporal. There were no significant differ-ences in RNFL thickness under low or moderate signal strength. Conclusion RNFL results are reliable under low and moderate signal strength.

Objective To investigate the value of the proximal colon resection in the operation of rectal cancer.Methods 143 cases of rectal cancer in our hospital were selected and randomly divided into observation group (82 cases)and control group (61 cases).The observation group was treated with the excision of the membrane of the colon,and the control group was treated with routine operation.The pathological report of two groups was analyzed,the number of lymph nodes resection and the positive lymph nodes were compared,and the incidence of postoperative complications in the two groups were observed.Results In the observation group,1487 lymph nodes were detected,the average number of lymph nodes was 18.1,the lymph nodes metastasis were 203,and the average number of lymph nodes metastasis were 2.5.No recent complications occurred.The control group of 61 patients,a total of 749 lymph nodes were detected,the average number of lymph nodes detected 12.3,lymph nodes metastasis were 103,the average number of metastatic lymph nodes metastasis were 1.7.No recent complications occurred.There was a significant difference between the average detection rate of lymph nodes in the two groups(P < 0.05).Conclusion The proximal colon resection can improve the resection rate and reduce the residual of positive lymph nodes,which can reduce the recurrence and metastasis of rectal cancer.
